Below are a variety of important forms that you may need to access and complete throughout the school year.

ALL Forms are required to be filled out in PowerSchool

In order to volunteer on a regular basis, we follow the guidelines of the Diocesan Child Protection Policy. Volunteer requirements:

 Health

Form Description of Use
Emergency Medical Auth. Required for registration - update annually and when changes
Health History Required for registration
Medication Authorization Required for administration of any medication during school day
Inhaler / Epipen Required for students who require Inhalers or Epipens at school

Registration

Form Description of Use
Registration Form Required to enroll new student for the 2026-2027 school year
Re-Registration 2026-2027 Required to re-enroll students for the 2026-2027 school year
Emergency Medical Required annually for registration or re-enrollment
Health History Required for registration
Legal Custody Required for registration - update when any changes
Public School District Required annually for registration - update when any changes

Form Description of Use
Internet Policy All parents and students must read annually
Internet Access Agreement All parents and students must sign annually (after reading policy)
Photo/Publication Permission Parents sign annually to grant/restrict photo and publication use
